How to create working form mail

Hello, several times I had tried to create a data collection form, but I never managed to figure out how to send the information to my email avoiding spam ...

Reading documentation from my provider's site, I understand that I have to use a "CDOSYS" object or a "Persits AspEmail component" .. but for both I can't find other documentation and I don't understand how they should be integrated into the html form page ...

Could you help me by providing me a very small example of html form with
- name (required)
- surname (mandatory)
- email (mandatory)
- telephone
- dropdown menu "your age: 0-10 ; 11-50; 51-100" (mandatory)

and the other file (only if need a separate file) "CDOSYS" or "Asp" which takes care of sending the collected data to my email "myemail@myform.yo" ?

I'm afraid the knowledge about Windows servers isn't big around here. I had never even heard about CDOSYS. I googled but didn't get very far. Sorry.
