I have used a perl script to create this form:

When submit is pressed it displays a plain white page with a thank you message, I would like this message to be displayed within the pages design. The script can be found here:

I used the compat version.

From the readme of the compat package:
CODE

redirect                - If this value is present it should be a URL, and
                          the user will be redirected there after a
                          successful form submission.  For example:

                          <input type="hidden" name="redirect"
                           value="http://www.your.domain/foo.html" />

                          If you don't specify a redirect URL then instead
                          of redirecting formmail will generate a success
                          page telling the user that their submission was
                          successful.
