I need someone to create me a website page.

It needs to have the following

<user> <bannedBY> <Reason>

I am using this for a game I play, and we need to accurately manage who is banning who.

So, when a USER is banned, it will show their username, then who they were banned by(The account you are logged in as), and the reason. This will be in a list format, seperate by a line, or a small image.

To add a user to the list, you need to have a USERNAME and PASSWORD to add them.

Hope someone can conjure me up something!
