Link to a website that has a form and autofill part of it, autofill a form that is already created on a different website Options

I am not sure if I am posting in the correct portion of this site. Here is what I am trying to do:
I am a reseller of a product.
They have a referral section on there site.
I host my own website on my server (not sure if that is important but thought I would through it in)
The referral section has a form that needs my contact info (first/lastname, address, etc...), my customers info, and then the names of the people the customer is referring.
I want to be able to put a link on my site and, when the customer sees the referral site, my information is already filled in. I can't just rebuild the site because their site issues a code that gets my customers $15 from the company that I sell the product for, plus we give an additional finders fee. The site that I am trying to autofill is: http://clearwire.dll1.com/ae/refer/

I am not sure if I need a script or just a really good link to fill in the fields that I want to fill in. I do not know scripting at all so if it is a script I would need either guidance or an example to go by (an example would be easier as I can just manipulate it and not have to keep asking questions). I would also need to know how to execute the script as I have never done that before either.
