I am taking an HTML class in my school. I taught myself most of HTML during the summer and because of that I am the top in my class. I know mostly everything except for frames and forms. I know the basics of frames but I never taught myself or learned forms. I'm sure I can learn how to create codes for forms on w3schools or something. One of my teachers knows that I am good at HTML and asked me to create a web page for our schools website. I am part of an organization in my school and my teacher wants to have a reunion for alumni of that organization. Because of that, she wants me to create a web page on our school's website where the alumni can enter the following information:

Name
Address
City
State
Zip
Home Phone
Cell Phone
E-Mail Address
Year of Graduation (As a drop down menu that has the choices 1990-2009)
A question with 2 check boxes next to it containing "yes or no."
Current Organization ________________
Current Occupation ________________
College Attended ________________
Degree Attained ________________
A box for additional comments

Submit Button

I really don't know how to create this kind of stuff in HTML. And, even if I were to create this, I have no clue how to direct where this information gets transferred to. Can anyone help me? I need to know how to create all of this stuff in HTML and how and where to store this submitted information. I have Microsoft Expression Web 2 and Adobe Dreamweaver CS3 available for use. Can you please help me?

You can have it sent to an email address or store it on a web server (in a text file or data base). If the latter, you also need to decide who can access the submitted data, and in which way.
