Using the login information you sent I get this error every time: "Connect failed: A connection attempt failed because the connected party did not properly respond after a period of time, or established connection failed because connected host has failed to respond."

In the mean time you should send a help request to your hosting provider. Send them the login information you use and ask them to verify it and ask if there is any other information required, like maybe a port number.

Ok, the problem turned out to be your database design. You were requiring every field be filled in. If every field wasn't filled in then the record would not insert. I changed a lot of them during my testing. You need to decide what data is required and what data is not required and setup your database accordingly.

Are you going to allow duplicate names/email addresses? If not then you should define some UNIQUE Indexes.
