Hello,

I have a simple form on my website for people to contact me, but the information in the Reservation box is the only information that shows up in the email I receive and nothing comes through from the other 3 boxes. Here is the web page:

Any help would be appreciated.

Start by fixing the HTML markup errors found by the online validator.

Then go see the FAQ entry How do I get form data emailed to me.
